XactGuide / XactStack™

One indexed system. Two guides. Zero guesswork between them.

XactStack is our prosthetically-driven, stackable guide system for full-arch implant reconstruction. Every case begins with the final prosthesis in Exocad — the bone reduction and implant positions are then engineered backward from that outcome, so both guides in the stack share a single fixation reference and drop into exact registration with one another.

Plan the smile first. Everything else follows from it.

EXPLODED VIEW — all three components index to the same fixation pins for guaranteed alignment through every stage of surgery

Three layers, one plan

01

Fixation Base

Anchors to bone or remaining dentition first, placing the fixation pins that every subsequent layer registers to. This is the geometric anchor for the entire case.

02

Bone Reduction Guide

Drops onto the fixation pins and defines the exact vertical and angular bone reduction required to create restorative space for the final prosthesis.

03

Implant Guide

Replaces the reduction guide on the same pins, post-reduction, and directs osteotomy and implant placement to the prosthetically-driven positions planned in Exocad.

Why prosthetic-driven, not bone-driven

Most guide errors trace back to a plan that started at the bone and hoped the prosthesis would fit later. XactStack reverses that: your Exocad predesign defines tooth position and occlusal plane first, we engineer implant position and bone reduction to serve that design — so the reduction level, implant angulation, and emergence profile are all working toward the same final result before a single burr touches bone.

Surgical procedure map

What happens in the operatory — step by step, in order.

01

Fixation Base placed

Anchors to bone or remaining dentition. Sets the fixation pins that every subsequent guide registers to.

02

Bone Reduction Guide

Drops onto the same pins. Defines exact vertical and angular bone reduction to create restorative space.

03

Implant Guide placed

Replaces the reduction guide on the same pins. Directs osteotomy and implant placement to prosthetically-driven positions.

Done

Prosthesis fits perfectly

Every guide indexed to the same pins. Bone reduction, implant position, and prosthesis all planned from the same starting point.

Why this matters: Because all three components share the same fixation pins, there is no re-registration between stages. The bone reduction level and implant angulation are locked to the prosthetic plan from the first incision to the final placement.

What we need from you

  • CBCT scan (DICOM) of the arch(es) being treated
  • Intraoral scan (IOS) of both arches, including any remaining dentition
  • Bite registration / occlusal record for prosthetic setup
  • Restorative intent: implant system, number of implants, desired tooth position or existing denture reference
  • Delivery takes 1–2 weeks depending on case complexity
